Transition Plans and Services
Part of a student's Individualized Education Program (IEP), the Transition Plan must be included by the year a student turns 16 (or earlier when appropriate). Based on age-appropriate transition assessments, the IEP Transition Plan outlines individualized transition services designed to help students reach their postsecondary goals.

T-Folio Curriculum
T-Folio is a free online transition portfolio tool for high school-age youth with disabilities. The curriculum aligns with Individual Education Program (IEP) transition planning, DVR Pre-Employment Transition Service categories, and the CCTS Transition Services Flowchart. Many activities also align with the High School and Beyond Plan required for all students for graduation.
